Important Information: Please read before filling out this form.

  1. Please fill out the fields in the form below. Fields marked with * are required fields so that we may contact you regarding your membership.

  2. By filling out this form, you are only submitting your information and request to be monitored by the National Sports Monitor. You will be contacted regarding your submission and then your account will be activated for monitoring once your contact information has been confirmed and your payment has been received.

  3. If you do not have a company name or website please put your handicapper name in the "company name" field. Your company name will be displayed on the "Capper Profiles" page for all visitors of the site to see.

  4. Your membership in any sport at National Sports Monitor automatically registers you in the Ultimate Handicapping Challenge for that same sport at no additional cost to you. The UHC will bring even more exposure for your handicapping business. There's no other monitor on the web that can match a deal like that. Two avenues of advertising for one low price.

  5. Feel free to use HTML in the "description" field and upload banners or files from your own web server. Consider your profile your own personal web page within the National Sports Monitor. Your profile will be seen by all visitors to the site and should look sharp and professional. You will be able to edit your profile as often as you like in order to turn visitors of the site onto certain promotions or packages that you currently offer.

      NSM NFL and/or NCAA Football $199
      NSM NBA and/or NCAA Basketball $149
      NSM MLB $99
      NSM NHL $49
      All Sports Discount (A huge savings) $299
      * Rates are per season and / or per calendar year if choosing the all sports package (all sports package includes a full calendar year of all sports beginning with the first day you start to play)

  7. Please note that upon receiving your NSM monitoring fee(s), you are automatically registered for the same sport(s) FREE at our sister site, the Ultimate Handicapping Challenge.com.

If I am found guilty of submitting different selections to NSM than those submitted to my clients, I am aware that my NSM membership will be suspended and/or terminated without notice.
